Brookmeyer and Gail (1988) also use a Weibull distribution to model the distribution of the time to incubation distribution used for predicting the course of the AIDS epidemic. 7.3 FLOWGRAPH ANALYSIS OF SEMI-MARKOV MODELS Flowgraph models provide a link between traditional multistate models and semiMarkov processes. Flowgraphs model semi-Markov processes and provide a data analytic method for estimating densities, survivor and reliability functions, and hazard functions for waiting times of interest. There is very little work on non-Markov multistate models (cf. Hougaard, 1999). Semi-Markov analysis is restricted to cases where the waiting times between states all come from the same family of distributions, such as the Weibull, or each transition in the multistate model is modeled by a separate proportional hazards model. Although semi-Markov processes are a natural extension for multistate models, in practice, data analysis for semi-Markov processes can be quite dif cult, especially when returns to a state are allowed (cf. Andersen and Keiding, 2002). Flowgraph models provide an extension of traditional multistate models to include semi-Markov processes. They allow vastly different distributions for waiting times between states, and they provide a closed form for the distribution of quantities of interest, such as total or partial waiting time in a stochastic network. 7.3.1 Flowgraph Model for Diabetic Retinopathy Retinopathy is the name given to a disease of the retina due to diabetes. The retina is the lm at the back of the eye, analogous to lm in a camera. Light enters the eye and passes through to the retina. The messages about what is seen are then passed on to the brain. People with severe diabetes eventually develop retinopathy, hence the name diabetic retinopathy, which eventually causes blindness. Figure 7.4 is a owgraph model for the degenerative stages of diabetic retinopathy due to Yau and Huzurbazar (2002). This model was rst proposed as a Markov model by Marshall and Jones (1995). The stages of retinopathy in the model are classi ed according to a modi ed Airlie House classi cation scheme (cf. Diabetic Retinopathy Study Research Group, 1981). Grade I indicates no retinopathy. Grade II indicates microaneurysms only. In this stage there is damage to the macula, the central area of the retina. Grades III and IV indicate intermediate stages of background retinopathy. These involve early changes, such as an increase in the number of microaneurysms. Grades V and VI indicate preproliferative and proliferative retinopathy, respectively. In preproliferative retinopathy, the retina has been damaged by higher-than-normal sugar levels over several years. This is the stage before new blood vessels start growing. In proliferative retinopathy, very small blood vessels grow on the surface of the retina. In this condition, the sight is very badly affected and can lead to blindness. State 1 in Figure 7.4 corresponds to grade I in the Airlie House classi cation, state 2 is grades II and III, state 3
