Patients with malignant ovarian GCTs or sex cord stromal tumors (such as granulosa cell tumors) found on frozen section can present a dilemma from the surgeon s point of view. Fortunately, the surgical approach is very similar for all histologic subtypes. The primary message is that ovarian cancers in young women should be managed in a more conservative fashion as compared to older women with epithelial cancers. Most patients with germ cell tumors are young (average age 19 years) and desire to preserve fertility if at all possible. Fortunately, most germ cell tumors are con ned to one ovary. Furthermore, given the extreme sensitivity of GCTs to chemotherapy with excellent survival results, we have found that extensive debulking (including a TAH, BSO) is not helpful. Therefore, the surgeon should plan to preserve fertility and avoid hysterectomy and BSO in these cases. In most cases, a unilateral salpingo-oophorectomy is performed, followed by surgical staging. Staging should include: r Complete exploration of the peritoneal cavity and palpation of pelvic and paraaortic lymph nodes r Washings from the peritoneal cavity are submitted for cytologic examination. r Care should be taken to avoid manipulation and trauma to the contralateral ovary and tube in order to preserve fertility r Additional staging should include: r Omentectomy r Pelvic and para-aortic lymphadenectomy r Excision of any suspicious lesions If laparoscopy was used to remove the affected ovary, it is acceptable to proceed with laparoscopic surgical staging if the surgeon is appropriately trained. For advanced-stage disease, laparotomy is still the approach of choice. Surgical treatment in women who have completed child-bearing traditionally includes total hysterectomy with bilateral salpingooophorectomy, bilateral pelvic and para-aortic

In the cross-layer approach, all possible interactions and integrations among layers are considered in order to optimize the entire system. Such an approach appears particularly valuable in the context of WSNs, for the high specialization of this type of solutions. Indeed, in this context, it appears reasonable to sacri ce the exibility of the layered approach in order to better exploit the peculiarities of the systems at every layer and design more ef cient solutions. In the following, we consider the main issues concerning WSNs, also taking into account cross-layer solutions, and we underline the protocols, algorithms and proposals that are particularly suitable for environmental monitoring.
