Perhaps the most compelling evidence comes from the INTERHEART Survey, a massive case-control study that recruited over 12 000 cases and 14 000 controls from 52 countries (Yusuf et al., 2005). This demonstrated that the risk of acute myocardial infarction rose progressively with increasing WHR and waist circumference; that this effect operated across a wide range of BMI; and that the in uence of WHR and waistcircumference was considerably greater than that of BMI (see Figure 9.7). The populationattributable risk (PAR) for myocardial infarction was increased by 27% in the two highest quintiles of WHR, while the two highest quintiles of BMI increased risk by only 8%. After adjusting for all confounding variables, including BMI, the highest quintile of waist circumference was associated with a 77% higher odds ratio for myocardial infarction than the lowest quintile. Also consistent with a dominant effect of central obesity, increased WHR (but not BMI) was found to predict a twofold increased risk of cardiovascular mortality in a prospective cohort of subjects undergoing coronary angiography (Hoe e et al., 2005).
