Lifestyle modi cation can be effective, but succeeds most convincingly in the resourceintensive setting of clinical trials. Even then, the vast majority of obese subjects fail to respond adequately and those who do will mostly return to their previous ways and weight when expert supervision and support are relaxed or withdrawn. Weight-reducing diets continue to proliferate, a sure sign that none really works. Against the expectations and prejudice of many obesity experts, one heavily-marketed diet now appears as effective (i.e. not very) as other more intuitive strategies. A hard look at why dietary recommendations succeed or fail has been urgently needed for decades, and this is still the case. We are now better placed than ever before to explore the physiological, psychological and molecular genetic determinants of dietary compliance and outcome. However, to do this effectively, we will need a large-scale, coordinated and dispassionate approach a combination that has so far proved elusive. Anti-obesity drugs may also have reached a watershed. When we rst drafted this chapter in mid-2008, three agents were established in clinical practice (at least in Europe) that, if appropriately prescribed and monitored, appeared adequately effective and acceptably safe. The excitement of checking the proofs was enhanced by the sudden removal, in October 2008, of licensing approval throughout Europe for rimonabant. The balance was tipped by further con rmation of a signi cantly increased risk of major psychiatric disorder, including suicide risk problems that had already been highlighted in earlier studies and that had persuaded the manufacturers to withdraw the drug from consideration for licensing in the USA (but not to discontinue its use in Europe). Neither orlistat nor sibutramine appears to be at risk of a similar sudden fall from grace, but all of these drugs fall short of the ideal which may in any case lie beyond the reach of pharmacotherapy. With each of these drugs, only about one-third of patients can expect to lose 10% or more of initial weight and virtually none will shed all their excess weight. On stopping treatment, weight and presumably the burden of comorbidities will return in most cases towards pre-treatment levels in those patients (the majority) who have not succeeded in putting their own anti-obesity lifestyle measures into place.
